The instrument was wrong.

Not because anyone was careless. Not because the stores team didn't try. But because somewhere between the booking and the collection, nobody asked the one question that would have prevented everything:
"Which coablator do you need, Doctor?"
That question takes two seconds. The consequences of not asking it took considerably longer to resolve — with a patient under anaesthesia, a surgeon scrubbed and waiting, and an OR team standing still while someone ran to the hospital basement.
This is what assumption costs in a high-stakes environment.

What Inquiry Actually Is

Inquiry isn't asking questions for the sake of it. It's the deliberate act of verifying your understanding before acting on it.
In high-stakes environments, the gap between what was said and what was understood is where most errors live.
"Did you say atropine or atracurium?" — one syllable difference, completely different drugs, potentially fatal if confused.
"Which knee are we operating on?" — asked before prep, not after.
"You said start now — do you mean right now or after the next step?" — timeline clarity that prevents a cascade of wrong moves.
Each of these is an inquiry. Each takes seconds. Each has the potential to catch an error before it becomes an event.

When to Inquire

The rule is simple: inquire at the point of ambiguity, not after you've acted on it.
If something feels unclear — ask immediately.
If an instruction seems inconsistent with what you expected — ask.
If you heard something and aren't certain — ask.
The moment of uncertainty is the moment of inquiry. Not five minutes later. Not after you've proceeded and something has gone wrong.

Why People Don't Inquire

Three fears stop people from asking:
Fear of looking incompetent. "I should already know this."
Fear of slowing things down. "Everyone's waiting. I'll just proceed."
Fear of challenging authority. "The senior said it. Who am I to question?"
All three fears are understandable. All three are dangerous.
Because the cost of not inquiring — in medicine, in aviation, in any high-stakes field — is always higher than the cost of a two-second pause.

The Culture Problem

Inquiry only works if the culture supports it.
If asking questions is treated as incompetence, people will stay silent. If clarifying is seen as challenging authority, people will proceed on wrong assumptions rather than risk embarrassment.
In high-performing teams, inquiry is explicitly rewarded:
"Good question — I should have been clearer."
"Thanks for checking — I misspoke."
"Excellent catch — let me repeat that properly."
These responses do something important. They signal that asking is safe. That pausing to clarify is valued. That the team's shared understanding matters more than anyone's ego.

In Your World

You don't need to be in an OR for this to apply.
Before sending an email: "When you said all stakeholders — internal only or external partners too?"
Before executing a task: "You said urgent — does that mean today or this week?"
Before a meeting starts: "When you said the proposal is ready — ready to present or ready to review?"
The question that feels unnecessary in the moment prevents the mistake that feels catastrophic later.

The Pattern

The best inquiries are:
Short — "Which coablator?"
Direct — "Did you mean X or Y?"
Immediate — ask when you hear it, not after you've started

S — Structured Formats: Apply the template. Don't invent clarity under pressure.
C — Confirmation: Ask each person directly. Group questions get group silence.
R — Rehearse: Pre-brief before. Debrief after. Both are non-negotiable.
I — Inquire: Ask before you act. Clarify before you assume.
